Household Information

Number of people who buy and prepare food together: Count children under age 22.
At least one person is age 60 or older: Check for "yes".
At least one person is getting SSI or Social Security Disability: Check for "yes".


Income Information

Total amount of monthly income, before taxes. Do not use "$" or "," in the number.

Income from jobs: Do not include self-employment income.
Self-Employment: Income from your own business. (Before costs)
Other Income: Examples: Social Security, child support, unemployment, alimony, pensions, interest.


Expense Information

Total amount you pay in one month. Do not use "$" or "," in the number.

Rent or mortgage: Mortgage includes property taxes and structure insurance.

Court Ordered Child Support: Child support that you pay for a child that does not live with you.
Medical Expenses: Medical expenses for elderly & disabled people in the household.
