This website shows results of investigations where the abuse complaint was substantiated. Note that this website does not contain information related to the following:
  • Licensing or rule violations,
  • Surveys (Facility Inspections),
  • Notifications of Findings (NOFs),
  • Letters of Determination (LOD),
  • Applicable sanctions, or
  • Multnomah County Adult Foster Home Investigations.

When exploring your options for a licensed facility placement, we recommend you:
  1. Note substantiated abuse investigations found with the help of this web page*.
  2. Schedule a visit at the facility you are considering and have a conversation with their administrator.
  3. Prior to or during your visit, request copies of the facility's most recent inspection reports.
  4. Contact the Office of the Long-Term Care Ombudsman (1-800-522-2602) and ask for information about the facility you are considering.
