Public copies of complaints are available for viewing at the local Seniors and People with Disabilities (SPD) and Area Agency on Aging (AAA) offices by appointment. Local office contact information and locations are available at: http://www.oregon.gov/DHS/spwpd/offices.shtml
To obtain additional details regarding a complaint investigation report, a request can be made using one of the following methods. A charge of $0.15 per page plus staff time may be applied.
